Practices that participate in common service agreements may also require their host organizations to facilitate and manage VBP contracts. To support the negotiation and management of VBP contracts, the host would need: with their limited resources, small firms are having difficulty providing the capacity to provide care as a medical home. The inclusion and payment of new staff, the training of existing staff and the development of patient services entail costs that can jeopardize the financial viability of small practices. As with a real cohort, many of these costs would be lower or even zero because a cohort of firms would not have to acquire all the services. To be useful, the model must be adapted to local realities and current capabilities and priorities of practice. The actual cost of a shared services program depends on several factors, including the basic skills of the practice, the type of patient population served and the administrative costs of the host. The PHIP report quantifies and evaluates the concept that small firms – which are disadvantaged in terms of hosting and participating in BPVs – can better position themselves by coming together and working with a host organization to share the services and costs of these services. It also provides a framework for practices and potential hosts that can be used when considering launching such efforts. Under shared risk VBP agreements, supplier incentives are generally calculated after the end of the fiscal year.

Then, it takes an additional six months to be the subject of late claims. To ensure financial sustainability, it is therefore likely that small firms will consider the gradual gradual introduction of common services and/or the pursuit of other sources of income. In theory, a group of small primary procurement firms could work together to create and fund a new organization offering these services in-house. However, the necessary infrastructure, start-up costs and operating costs can be prohibitive. A more viable way would be to enter into a contract with an existing organization – an independent practice organization (IAP), a large medical group, a hospital or a health system – or to work as host of a number of common services. There are some potential sources of investment to cover the start-up costs of implementing a shared services program. For example, a source of investment capital could be to allocate unspent funds under the National Innovation Model (SIM) or the Delivery System Reform Incentive Payment (DSRIP) program, which underscore the importance of a transformed primary procurement system. However, common services also require the second form of funding – extended payments to compensate for the practices or host organizations involved for their increased operating costs. Editor`s Note: Primary procurement firms are under increasing pressure from payers and supervisory authorities to move to a home-based medical model and to participate in value-based payment contracts (BPVs).

The necessary changes – new employees, skills and capabilities – increase operating costs in a particularly heavy way for small primary supply firms. A new report, prepared by the New York City Population Health Improvement Program (PHIP), in collaboration with the United Hospital Fund, outlines a possible solution to this challenge: common service agreements that would allow small primary supply firms to successfully adapt to the new VBP world. The report below outlines a financial and operational model for small firms to share the cost of the services and capacity they need to thrive in the new realities of health care and funding.
